HTML form (self calculating) to PDF creation used on Joomla website

This is pretty simple but I am looking for someone with an eye for detail and style. Please see attachment for general rough draft of form to be filled out by customer. There are several self-calculating fields that should be instant and there may be a few last minute changes that need to be done in the end (so be prepared).
The customer fills out the various fields on our website and then the data should then be merged into a pre-designed PDF form. The customer will then have the option to print the form directly from the website (without a PDF viewer - is this possible?), save a PDF version on their desktop or have the PDF emailed to their email address.
This is to be used in a Joomla 2.5 website, so it needs to be easy to incorporate, without losing the website framing, etc. Also - no cookies should be needed. Very important -- no cookies (as I've had issues with Joomla and cookies in the past).
I'd like to see a small online example with your bid, so I know you have experience with this type of setup.
