Build a fillable PDF form for system user access

I need someone to build a fillable PDF form where I can send to my users to fill out when they have a request for system access when employee are starting. This form is very basic (fields for employee information, bullet selections, maybe a drop down). I want the form to be editable so that as the business changes my team can make changes easily. This is a great simple starter project so we can build a relationship. Additional points are given to those are able to pick up quickly on what I want to accomplish, reliable/consistent communication, and bring input/creativity to the project.
