redesign an ionic 1 app

I have an ionic 1 app with a very basic design (starter app design). Needs to get it redesigned with a more modern look and feel. This is an easy task for the right person, only a couple of hours of work The design need to be consistent and more beautiful, but keep in mind that this is a tool for field service reporting so don't "decorate" to much . You may use a theme pack if possible. I like the material design, but are open for better ideas. Requirements: 1. See the attached pdf (It is my thought, but I'm open for better ideas se Requirment: 4) 2. The app should be used on a tablet up to 12" and the design should fit all screen sizes up to this. 3. I don't like that my fields (label and textboxes) fills the whole with today, so please fix this according to the attachment. 4. I expect that you suggets better design for labels and buttons. I'll pay bonus if you impress me and deliver more than I have described in my requirements. 5. I also will need an appropirate icon to the app, this could be an icon of a bug of some type 6. It's important that the design through the whole app is consistent 7. Use the companys color profile in the design I'll send you the company logo that should be used in the app when I have awarded you the project.
