Convert a Scanned PDF to a Microsoft Word Document

I have a 600DPI scan of a 4 page document. I need a Microsoft Word version of this document created with all the printed text on the form and all the handwriting removed. The Word document should look like the form before it was used with all formatting in place.
I have attached the scan of the document and also an attempt at OCR of the document. You can use the OCR attempt as the base of the document if needed.
There should be no scanned elements remaining in the final document, it should be all native Microsoft Word text elements.
