interactive clinical notes

I would like to have an interactive intake form for my acupuncture clinic that can be securely stored on a usb or hard drive as a editable file. Also basic follow up form where I can type in changes from last treatment and other notes. I would like it to have five element charting, body clock charting, pulse qualities ,a body pain chart with a Likert scale and qualities of pain and worse for... or better for...sections. it would have lists of symptoms organised into their relevant organ networks. What I would like to be able to do is have all of these things in the form that I would be able to touch with my finger to check off instead of having to write down too much during my consult. The other part of this form would include an editable by writing section for filling out the usual patient information name address etc and medical history,allergies and medications and an informed consent section that can be signed electronically.I am just interested in enquiring if anyone is capable of producing this for me at this stage.
