AngularJS Modal Value Picker

Hi, This is a request for AngularJS modal/ng-table/bootstrap functionality. Please make sure you have read and understand all requirements before bidding. I need a form with a Bootstrap input box on it with an icon attached to it. When icon is clicked, a modal window should open and display the attached json in a table format that is filterable. I would prefer you use the Angular ng-table directive for this. I would also prefer the Angular Modal Service is used as well ([login to view URL]). With the attached JSON data, the sys_id field should NOT show in the modal table, but when a value is selected from the table, modal should close and the "name" field of the JSON should populate the input box and the "sys_id" field of the JSON should populate a scope variable NOT on the form. Attachments include JSON data and screenshot that closely resembles what I am looking for. Expected output is AngularJS project I can open and run locally with no issues. Thanks!
