build an android car inspection checklist --

I have a .docx file of the checklist (word file), i'd like to convert it to an android app.
i want to be able to add pictures to the checklist
and to be able to print it to pdf and send it via email
i would like to have a sequential number for each time i use the sheet
i would like to have the option of using the location tag and date and time stamp
i want to be able to fill in multiple checklists off-line, then when i get back home at the end of the day, to be able to send each one to it's respective email address
I want to have a checkbox (for customer to agree with terms and conditions) and a signature section to be stored for each individual inspection (to be able to fill that information off line)
Basically to be able to populate several inspection sheets offline (and having each customer agree to the terms and conditions, and sign... different inspection sheets are different customers) then to be able to send the inspection sheets to the customers when i get back home at the end of the day.
I would like to also have the end product to be able to install it on multiple android tablets and phones.
I attached a PDF of what the file should looks like. I have also attached an empty sheet for comparison (to have a general idea of which fields i usually edit while filling out the inspection sheet)
