Design two forms using Contact Form 7 and MailChimp that look attractive on my site

I need two forms designed to fit the look and feel of my site at [login to view URL] It's a Wordpress site and your design would provide all the code I need to insert in my Contact Form 7 screens and my Mailchimp screens. You will also provide the CSS necessary which I will insert in my child theme. You will not get admin access to my website so you will have to design it on your own Wordpress installation and provide me the code. Note: You will also need to have a Mailchimp account to complete this job. If you feel you have experience in EXACTLY this job (not general webdesign or WP or anything else) , provide some examples of forms you've created. DO NOT GIVE ME LINKS TO ANYTHING ELSE DO NOT GIVE ME LINKS TO YOUR PORTFOLIO OR OTHER DESIGN JOBS YOU'VE DONE But make sure you give me links to FORMS you've created. AND convince me you are very proficient in Contact Form 7 and MailChimp. This is vital - if I do not see immediate demonstration of expertise in these two particular programs I will ignore your reply.
