Convert a One-Page Coming Soon Template to a One-Page Website

I have purchased and downloaded a one-page template from ThemeForest. This template features a video background (video included), a countdown clock, and an email address submit box. I am in need of a Freelancer to:
1. Upload the Template to my Hosting Server.
2. Ensure that (i) the video background will play correctly (ii) that when a visitor enters an email address and clicks the Submit button that this email address will be forward to a designated email box; and (iii) need my business’ address and phone number placed onto the page above the email Submit button.
3. I have already adjusted this template so that (i) the countdown clock is set for forty-days; (ii) copyright information is changed to reflect our company name; and (iii) inserting the video path location into the style sheet.
4. Test web-site once uploaded to ensure that everything works.
5. I am asking that the Freelancer provide me with a final web-site using my purchased template so that when these components listed above are used that they will work as designed.

Upon acceptance of Posting you will be provided: (i) the login information to access the hosting server; (ii) a link to the Dropbox Folder that will have the template and any additional files or information, including address and phone number of business.
