Basic web template

I need a simple template designed. I want a header graphic at the top of a white, rounded corner box with drop shadow approx 650 wide. If it should be wider, please let me know. Site is a help desk. Please see attached images. I will add in title to the graphic, so please leave room for title. I need the files and also a psd photoshop file. I will only respond to people who send me a mock of the job. Your previous work is great, but has no bearning on this job.
