Appointment booking application and website development

I am doctor, I do private practice in different places. I want a website build for myself. I t will work as Website and also an application. Here are the specifications of the application. • I should be able to add/edit/delete these addresses. • I should be able to add define my schedule for each addresses. • When a patient visits the website and clicks on the Book Appointment tab he/she should be able to see my schedule both date wise and location wise. • Patient should be able to book an appointment for a particular location for the already defined timing. • The development team has to create the design, layout the make the make the necessary images also. I will provide the contents.
