Create a coupon page with PHP/MYSQL

I need a coupon page created. It will need to be designed for display and an admin page for entering the details stored in a db. It will have a headline field, a text field, an upload image area, name and address of the business and an expiration date. Should be a pretty simple job for someone who knows PHP/MYSQL and a little design skill.
