Access Database Project

I have a database in Microsoft Office Access 2003 that I need to be developed so that I can add and delete records, add additional information into each record, and create custom mailing lists. The database currently has 725 records with about 40 fields. Each record corresponds to a program in a health care treatment organization. It is important to understand that each record is a health care program and some organizations have multiple programs. As I find out more information and attempt to contact each organization about their program(s), I would like an easy to use method of recording the information I'm collecting, creating a mailing list to mass mail letters, keeping track of who I've contacted, adding new organizations or programs that I learn about and eliminating organizations or programs that I'm no longer interested in. Here's a list of what I want: 1. Add fields for each organization so that I can record: (a) general information and notes, (b) Contact names, title, phone numbers, e-mail addresses, (c) letters mailed and telephone calls made, (d) organizations eliminated including explanation. Also, add a field for each program so that I can record general information and notes. 2. Forms to allow: (a) addition of new organizations or programs, (b) display and update all information about an organization and its programs. 3. Filters so that it is easy to view: (a) organizations that have programs located in a certain U.S. state, (b) organizations that have programs that I have not yet eliminated from my research, (c) organizations with programs that have an "X" in a certain field (d) organizations to which I have or have not sent a letter. 4. A capability to generate a mail merge list for Microsoft Word 2003 based on the Filters described above (preferrably a list that is in Microsoft Excel 2003 format). ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform The work is to be done in Microsoft Access 2003 with a mail list to be exported to Microsoft Excel 2003.
