App to log Time/GPS/User fields to online SQL database

I need this app developed for IOS VERY urgently. The app will eventually need to be developed for both IOS and Android phones, so please indicate if you can do both in your bid and how much you would charge for both and how long it would take. I have attached a VERY clear specification document so I DO NOT want to PM, email, skype or chat with you about anything unless the attached document is not clear. It is a relatively simple app with only 7 "screens". I will supply all images to your specifications, an empty SQL database and FTP access - you need to do the rest. IMPORTANT - Money I will pay the following milestone payments and NO OTHERS. Do not ask for anything up front or any other payments. 25% at first working draft 75% at completion PLUS an extra 20% if completion occurs before the promised deadline.
