Contact form, Header Change + Widgets for Wordpress blog(repost)

I have a wordpress blog already up and running, I need the following done: 1. The header and footer need to have rounded corners (they are currently straight and square). 2. I need a contact form, with 30-40 fields and a logo upload field. 3. Adding widgets, to manage ad code. 1 Widget above first post, 2 widgets on the right side bar next to posts. I hope this can be done and finished today or tomorrow morning at the latest, it should be an hour or so of work.
