Contact Management - Phase 1

I am looking for a skilled, efficient, responsive coder to build a PHP, mySQL, Linux based web application to manage leads. This is Phase 1 of a larger Contact Management application for a specific industry. Act!, Goldmine etc. don't do justice and provide too much functionality the industry does not use. Phase 1 of the build (which this bid is for) will cover the following functionality: - User Registration screen (5 fields with logic in 2 fields; 4 user types) - Screen to capture first interaction between customer and user (10 to 12 fields; downstream PDA integration will be required so if you have skills in this area that will be an added plus; if not, no worries, we can solve the problem when we get to it) - Lead Metrics summary screens for 3 user groups (with 3-6 fields per summary screens) More detail within/about the above parameters will be issued to selected coder upon signing a NDA. Please only respond if you have done exactly this kind of work. Please point me to examples of at similar web applications you have built. I will make my selection based on the quality of your work demonstrated in the examples you send me. Bid evaluations will take place between January 8th and January 15th. The bid will be awarded on January 16th. All development should be completed by January 29th. Testing should be finished by February 4th. Launch will occur soon after. Payment of fees and bonus will be paid at that time. Phase 2 build requirements will be issued in early March. Thanks in advance for your response. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform PHP, mySQL, Linux, AJAX
