Program Daily appointment scheduler Monday to Satursday Appointment every 20 minutes Two operators The clients either book for one (1) session cost $88 Three (3)sessions cost $231 Six (6) sessions cost $396 Ten (1) sessions cost $550 If they book and pay for more than one (1) session we need to tell the system the amount they have paid. And every time the person uses a session the system should keep track of how many they have paid for and how many the have used. The system should alert us to the fact that they have used all the prepaid and are now due to pay one more. There needs to be ability for the end user to adjust the session price from time to time. Simple data base First Name, Last name, Address, suburb, post code/zip, State (three letters not two), Country, Amount paid, Balance. Home phone, work phone, Mobile phone. Value added tax 10% is included in the amount paid and a field for vat should be included. So therefore if the client paid $88 then the vat included would be $8 And with $500 the vat amount would be $45.40. This need to be calculated for a month as a total. Printable table by all clients, groups of clients or single client. Selecting which of the fields are to be included in the print out. System should produce a receipt for the client ## Deliverables Problem/Opportunity Statement To build am appointment scheduler with a data base attached to keep a record of total visits paid for, the number of visits used and a flag to indicate that the pre paid visits are about to run out. Functionality (1) Simple appointment schedule (2) Personal client data base (3) Client financial info (4) Break up of payments made with totals of net payment to the business and the net payment (10% vat for the government) (5) Ability to print out at any time a full list of clients that have presented that week or month quarter or year and the $ totals at the bottom of this list. (6) With a client pre paying the system should keep track of this and when each appointment is made the system should indicate what number this is from the pre paid total i.e. ten appointment booked, one delivered to day system should indicate (9) indicate nine left. (7) At the time of payment system should print out a receipt. The format of this receipt should be able to be altered in the future. Constraints Budget is limited as this is a very small start up one-man business. Time the proposed start date of the business Jan 3 2005.
