Contact Form to submit guest list

First my common rules for working with someone.... - Needs to be available for instant chat (MSN) - Needs to be a reasonable price considering that i will be helping along the way (just don't have the time to do this all myself) - Needs to provide time frame and stick to it !!!!! - Make any bid personalized otherwise they will be ignored *********THE JOB********* I basically want people to be able to submit a guest list for a party online. I will have different venues with different partys and i need a form on each party page and then one master form. So i need to be able to update this myself and add parties, venues and forms. I also need each guest list emailed to me and to the person who submitted it. Fields: *Name - text field *Email - text field *Birthday - drop down list in 3 parts (day-month-year) Event Choice - drop down listt Guest List - text box * - This field will go into a database - The details that have been entered will need to be emailed to me and also to the person submitting the form - There will be a confirmation for the person sumbmitting the form "Your Guest List has been submitted" or error message if they made a mistake, this would be prefered as a pop-up. - No html/code is allowed to be submitted anywhere in the form - The subject for the email being sent will be "Guest List for [event choice]" - I would like the form to look like the sign up form on [login to view URL] but just different colours and smaller fonts and will fit in with the colours and feel of the website and be contained in it's own div named 'guestlistform'. Doesnt have to be the same size just the same feel (i think i need the width about 230px). I think this has covered nearly everything but i probably forgot a small detail or two, and your feedback on the best way to do this is welcome if it is better than how i pan to do it.
