Build a Web Form

Hello, I a interested in purchasing a web form from but I have a few specifics for this form that I need listed below. I plan to build on what you create. This is going to be an e mail form so once the user completes their information, they click a button to submit the data to a recipient. 1. The top of the form is going to have 5 fields to enter information. 2 of these forms need validation that they come from a specific e mail domain. This is where the user will enter initial data. 2. The next step will be a pick of 5 radio buttons. Once the user click a button, the 1st drop down menu will appear. 1 of the radio button will not require any additional info to be submitted, just click the button to send default info. 3. Drop down menus will continue to appear when a user has selected an item from the list. The menus will differ depending on what option the user selects. I've included a file that explains the drop downs. 4. Once the user reaches the end of the form, they click a button to e mail the data Can you do this?
