Chemotherapy tracker

This is much like a project manager or project calendar manager, but will track doses of chemotherapy and the timing of administration for specific patients. Key components: 1) Database (simple)-Patient demographic data: name, DOB, ID #, dosing specifics such as height and weight/BSA. Additional data for an active patient undergoing treatment woudl also list specific doses, labs and reaction. 2) Database-Chemotherapy regimens: drugs used, standard dosing, timing for each drug, anticipated number of cycles 3) Calculations-a patient would be selected and a treatment regimen selected. Based on the data from the above, a calendar of treatment dates, doses to be administered for each cycle and for the number of planned cycles would be generated. 4) Output would be both to print a simple but attractive calendar specifically for that patient that lists the planned dates of treatment. The specific layout of the calendar to be decided. Calendar would have the options of single month, 3-month, 6-month or year. 5) Additional output would be to a Chemotherapy flowsheet for an individual patient: lists treatment dates, doses actually given on which dates, labs for those dates and reactions. More details in enclosed zip file, plus willing to discuss what is feasible and what is not. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform To work in Windows 98, 2000 XP as well as on a tablet XP. Prefer to be in VB6, but am open to suggestions and ideas. Accepting of use of MS Access.
