Database for tracking specific patient information for a Dr.’s office. Involves approx. six many-to-many relationships. MUST be written in VB.Net using industry coding standards, including proper code commenting and documentation, naming conventions and such. A database design is in place but changeable if needed. The program needs to track specific information about the following: The patient, their listed contacts, their employer information, insurance information, their injury(s), their prescriptions for those injuries, some referring physician information, and either link the patient record to related images and sound files or store those images and sound files within the database itself (preferred). The laptops the Dr.’s office uses have still/web-cams and microphones built in so we need an image capture component developed for the program as well that can capture the image/sound directly into the related patient information schema when possible. The ability to import or link to pre-existing images and sound files must also be built in. It also must produce some specific canned reports/paperwork, but also let the doctor's office design and specify their own reports if needed. Database must support user level security (require login), must be multi-user capable (up to 20 people possible but 7 currently), and be run from a remote "server" machine and have each client attach to it over the wireless network. The system must also support replication from users that take information to the field on laptops and need to synchronize with the main database when they return. The MSDE database may be used if that becomes necessary for performance reasons, but all distribution/installation requirements still stand. Use of third party controls for this project should be minimal, but if used, full disclosure of the specifics of those controls, such as company, control type, version numbers and so on, is required. Preferred third party controls are ComponentOne, Pegasus ImagXpress, DataDynamics ActiveReports or Crystal Reports, or Infragistics components, so please try not to use any other “brand?? unless you have checked with me first. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform System OS Windows 2000, Windows XP Home & Professional.
