database website - import xls file -> export .txt file(repost)

I need a website finished that has several? tables and fields related to each other. The site also imports a .xls file (same field format each time) and exports a formated .csv and .txt file - in a specific field size and format.? I've attached a sample file that is uploaded, and the resulting files that are then downloadable. I've attached 2 .pdf files that show all the tables and related fields that I can find (note it may be incomplete) and a document with several screen shots of the existing site that we use now - the site is being shut down so we need to duplicate this on our own server. I can provide visibility into the existing site via webex screen sharing as needed. The site also needs to do some reporting out of the database - there is a screen shot in the .pdf of the defined report buttons - I can provide those samples as needed.? I think they'll be pretty easy though.
