Contact Form

The contact form I need created is the same as the one called "best contact form" ([login to view URL]) so rather than writing up every feature here, it will be best for you to go to the website that sells best contact form and look at it. This needs to have multi-part form capabilities. There are 3 video’s you can watch as well that will tell you exactly what I want. It should email and use mysql to capture the data. It needs to have the capability to show the same data as is described in the smart zone area of the sample website as well. You will most likely be able to get this data from the Google Analytics API. The code needs to be clean and professionally done. I will run this on my own server on a membership basis. Include the GUI interface with this price. I will create the website that will market the product. I just need the code, functionality and features. Once this is done, I will then want to create a Wordpress plugin and Joomla extension that will enable people to use the basic service using this contact form. Your input and suggestions on functions and additional features are more than welcome. Thank you.
