CUSTOM FORM

We are a screen printing company that is in need of a quote request form (or order form) for our clients to send us their job requests. This form would include drop downs, radio buttons, text fields and progressive drown-downs if possible. (where each drop-down calls up a different list for the following drop-down) Additionally - it would be nice to have the capability for clients to upload .jpg samples. This form needs to be hosted on our server (public hosting). ## Deliverables Basically we're open to suggestions. ? Ideally a 3-page progressive quote request would be best (with sections as follows: ? 1. shirts ? 2. artwork ? 3. customer info - submit) This form needs to generate 2 emails after it is submitted by a user - 1 confirmation email and one email to the business so a quote can be assembled. This form needs to be hosted on our public servers along with the database as opposed to being produced via a "form hosting site". The website that this form will be on is running the latest version of WORDPRESS. As I am unfamiliar with the amount of work that goes into FORM CREATION - I am aware that we may be asking too much for too little - please mention if this is the case. ? Thanks!
