Creating Custom Results Block in Moodle

1) We are using Moodle for student portal where we need to have a Results block that will lead to results for respective students (who has logged in with their account id).
2) The Block will also have another view like admin view or teacher's view where the teacher can select a particular student id from a drop down to get his/her results (results history).
The results should be in the attached format (semester wise).
A query to show results is already attached, you will have to modify it a bit to show the desired results format.
We are creating a block for this purpose because Moodle offers permissions facility.
The results database is also attached, you can simply add them to your Moodle project for testing and work around.
