Convert javascript to phpnuke

I need to convert the following quiz to a phpnuke´s module [login to view URL] Currently it is written in javascript (it is encrypted)(it is inside a IFRAME in this php page. Attached there is the script to take a look. It is not working because I did not attach all the required files (as, for eg., the java that makes the graphic), but the main script file is this one. I will send all files to who i accept the bid. I need only one thing more: to ask user´s e-mail, and put inside my 4nnewsletter block. But thats easy... just add an item in a MySQL table...
