Appointment scheduling program

I am looking for somebody to build an appointment scheduling program for my website. I currently use appointmentquest.com. I want the customers to be able to book the jobs inside of my own website which is www.aperfectshine.com. I want the program to basically be an extra page in my website. The program will not have to be as complicated as appointmentquest.com. I just need it to look very professional, clean and EASY. It would have a list of all our services. ""Click on service" to make appointment. Then when you click it would ask if you are a returning customer or exisiting customer. Returning customers would already have their information stored in a database. New customers would fill oout the information we need. The program would need to have the ability to enter in a credit card and to make sure it is valid. The software must also be able to send out automatic emails to both the customer and our company. The software should also have a backend where we can see the appointments and the database for all the customers and when the customers sign up for a job perhaps the ability to automatically send to our constant contact email list. I would want to work with the programmer on the design and functionality. I would basically tell you what I want it to basically look like and do and you would do the rest.
