I need some very quick simple work to modify the attached form to do the following: If a sport is chosen in the fall row, then the donation box should be set to $325, if none is selected it should be set to $0. If a sport is chosen in the winter row, then the donation box should be set to $325, if none is selected it should be set to $0. If a sport is chosen in the spring row, then the donation box should be set to $325, if none is selected it should be set to $0. The total donation box should be the total from the fall, winter, and spring donation boxes. A user should be able to update/change the amount in the donation boxes, which should update the amount shown in the total donation field. The total scholarship field should be the amount less that the default amount is select. For example, if only a fall sport is played, but the donation box is changed to $300, then the total donation would be $300 and the total scholarship would be $25. If a fall and a winter sport is being played, but $0 is entered in the donation boxes, then total donation would be 0 and total scholarship would be $650. Total Donation and Total Scholarship should be hidden fields that can be submitted with the form.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):
a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
