Achivements block for exsitisng project

I have a poker rankings site that I would like an achivements block added to. This will show the users profile avatar with earned achivments underneath, then there will be another section for thier "nemisis" avatar (this already exisits in the program) The achivments icons will be supplied by myself so all I would need is the coding of the block (see attachment for how?
