Build a Website

Very simple project. Looking for a programmer with experience in building databases in MySQL. I need a simple database of information that can be created, edited and searched from within a website. The information is split into 3 small sections- Job/Position; Candidates; and Questionnaires. The 3 sections contain the following information: Job/Position:- Job Title; Job Number; Job reference; Company Name; Candidates allocated to it; Date created; Close Date; and Job Still Open? (Yes or no). Candidate:- Name (First and Last); Email address; Job/Position(s) allocated; Questionnaire; Date Created; and Response received? (Yes or no). Questionnaire:- 5 Questions (created by user); Jobs/Positions allocated to it; Date Created Each candidate is allocated to a job (perhaps more than one), and each job is allocated to a questionnaire (only one). I've uploaded a fairly rudimentary diagram of how I would like the search to look. It's not exact, but you get the idea. This is paid 100% upon satisfactory completion.
