customer forms and database

Bascially I would like to set up a free web site where people can enter their medical information and it can be stored and then later updated or edited. I have designed some text areas using frontpage 03 and have tried to coincide these with a database in MS Access. However, i cannot get the two to work together. I am also not sure of how to display the information even if I did get it to store correctly into the database. I was thinking of creating a pdf type form or some type of form that people can edit later if they want, where they can view all of their results and print them out if they choose. This seems like a really simple project to me, but I have had a lot of grief over this. I have the web site done at home, and would like to just paste the appropriate code into my site pages (the entry fields are spread over three different pages), load the correct database to my server (that would recieve either a pdf type form or data directly into the three tables I created), and get started. I also am working on the password/username part based on some examples I have found on the internet.

So, I need help either creating new entry forms that link to a database that the registered user can view and update later on,,,or, help making my existing entry fields work with my database and tables, and some method of the registered user being able to display this info to print it out,etc later on if they choose.

Last but not least, I would like to be able to store some pdf documents and some images under each person's username. they would send them to me, I could convert the documents or images into pdf, etc (if not done), and I would then upload them into the database and have it also able to be viewed by the user when they choose. I suppose we could have it automated and they could upload it themselves as well, and maybe have an email sent to me that it was uploaded...but that is the basic idea.

## Deliverables

1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.

2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):

a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.

b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.

3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).

I currently use windows xp. my web site is designed using frontpage 2003. I would like it to be useable by people with the most popular browsers.
