Create an installation script/page

Creation of an Installation script. The coder will create a working installation page with a form to collect information and then install information to a Mysql database and additional information to a single page on the server.. These are the things the installation will do: 1. Collect the following information URL of installation directory server, Database name, Database user, Database password Licence Key email address Populate a MySql database with the database information:- Add the URL, database information, Licence key and email address to a single file in a directory on the server. There is a single file on the server within a directory. The directory is called "include" and the php page that all the above information must be entered in to is called config.php. /include/[login to view URL] I want a working installation page in php. A simple form with submission. I have provided the actual [login to view URL] file that is used. This is the file that all the information must be added to.
