Create Landing Page

We need a landing page, on which visitors can sign up to receive a Free Guide. They will need to provide their name, company name and email address to receive the Guide. Once the user hits Submit, we need to: 1. generate an automatic email that provides instructions for them on how to download the Free Guide (a pdf document). 2. capture their email address for future communication/marketing to the visitor. We have a mockup of the landing page we need created (attached). We are open to variations on this design if you have suggestions that make sense. For example, we will want some small links near the bottom back to our main website. Once the page is coded, we will need it uploaded to our server. The coding must include the necessary protocols to generate the automatic email response and e-mail address capturing. The actual page will differ from what is shown here - different images may be used, etc. Also, any suggestions you have for making the page more effective will be considered.
