Convert a psd

Hello, I want to convert a very simple psd into CSS/HTML. See attached As I don't have enough time for this job and I want to make sure that you understand the requirements before we may proceed. So please answers these questions. 1) Are you flexible for minor changes? 2) Do you have experience of coding Accessibility options. More information would be provided in private message. You should have command on JAVA Script. 3) I want a clean code, well documented CSS and html. You can use tables. 4) You should be able to provide me the first page coded in 12 hours after the bid is accepted. I need this job done in 36 hours, so please ask questions if you have. 5) The payment would be released after testing the code and functionality of the site. I want this code working same in all major browsers. I am very corporative and descriptive but I need a guy who can understand my requirements well and perform fast.
