Ballot box registration tracking

We are needing an application to track the entries from various ballot boxes placed around town. Pretty straight forward with the following frills. A database with items like name, address, phone. And one special field "ballot box location" The "ballot box location" field would be populated from the "location" database, thereby restricting the user to one of those entries. City and state should auto-fill from a supplied "zipcode" database. Other considerations; time, date and operator stamp for each record as entered. Need ability to export records that have not previously been exported.(emailing these to telemarketing person) Need ability to print labels for records that have not been previously printed. (printing labels for the initial mail piece) ability to export all records ( for monthly mailing ) lastly.. need a few reports. nothing fancy.. just some counts and averages for entry operator ( i.e.; Joe enters 14 ballots per hour and 75 per day) Location #1 brought in 105 ballots for the month of June... I gave this to another coder almost one month ago. Need ASAP. Bonus to fast coder. See attached zip for more detail. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition), completely self contained on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. 4) Networkable via windows 95,98,2000,XP. ## Platform Two installs expected. Installing to win NT 4 server with workstations running XP, 98. Also installing to win 2000 server with workstations running XP pro.
