Calendar Php,Ajax,Mysql

I need a web calendar written in Php,Ajax,Mysql for a Doctor's Office. The tipology of calender is like google calendar also the design of template . The view is for day,week and month and at the top You can select with checkbox if you wanna see the calendar of DoctorA,DoctorB and etc, theese doctors come from a query in mysql ,also is possible multiple selection if you wanna see togheter the appointments of DoctorA and DoctorB or others. When you click on the calendar open a window where inside there's a select name where you can search a patient in a database with ajax and if there isn't a patient you can insert a new one clicking on a button "New Patient" and when you've clicked on it appear five white text filed where you can insert (name,surname,phone1,phone2,email). If the patient exist when you select it with the select name automatically with ajax fill the thext field with (name,surname,phone1,phone2,email).Also there are two radio button where if you click on first"1 Visit" enable a select name with the default duration of 1 visit and this duration come from a mysql query because is refer to a doctor. And if you click on second "Control" enable the second select name with the duration of control also this duration come from a mysql query because also this is refer to a [login to view URL] are two other buttons and where with the frist you can save the appointment and with the second you can cancel the [login to view URL] You click on the frist or on the second the window close and automatically refresh the page with Ajax and you can see the situation of [login to view URL] doctor have different color.
