Developer needed for survey database design

MAG Myanmar is in need of an Access database to track the impact of our work in educating people about the risks of landmines. Reports will need to be produced which dis-aggregate the data and track key indicators over time. MAG Myanmar has developed a survey that will given to hundreds of respondents each year in order to track the impact of our work. In order to facilitate the analysis of the results, we need a functional database to track all the answers. The surveys are conducted on paper in the field and then entered manually at a later date. Retention of key messages that the survey tests will also need to be tracked. Respondents will retake the survey 3 to 6 months after the initial interview. Therefore, we will need to locate the appropriate respondent record and finish the data entry several months later. We need you to design the database from scratch using Microsoft Access. We will provide the survey questions and responses as well as a matrix for what information needs analysed. You will need to create the forms and reports that the end user will see. Since the data entry will be completed by staff with minimal technical skills, the interface will need to be seamless and logical. This is not a complicated job; intermediate knowledge of Microsoft Access and VBA should be enough to get this done. However, the design is key because the database will be adapted later for use in other countries where the questions and responses will vary.
