custom parser for node-csvtojson

I am attempting to parse a csv file into a custom JSON format.

I am using node.js with [url removed, login to view]

In the attached files is a CSV file of source data. Also, attached in an example of what the final JSON format should look like.

Please note that the sample output is hand generated by me. The final output would be of equal rows to source data. (50 rows of data in, 50 rows of data out)

First two rows of CVS file should be discarded (health and age). "Attained age" is used as the key for each new row.

final solution would be invoke on command line as "node [url removed, login to view] ./[url removed, login to view] > [url removed, login to view]"
