Autosuggest Form Input Field for Australian Postcodes

We just need a simple autosuggest/autofill for a location field. Users should be able to enter their Australian postcode or suburb and a list of suggested locations automatically comes up after 3 characters/numbers are entered. We will supply the location data needed. It should function very similar to the attached screenshot. Users should only be able to choose from the selection and not be able to enter their location manually. The input values will be three different fields in our database so they will need to be separated by a comma so we can split these server side (we will do this). eg <input type="text" name="location" value="East Victoria Park, WA, 6101"> Thanks.
