Code Form for Online Order System - Have Design Ready

I have PSD files for the Online Order System Form. The form has: (Find the PDF file attached) - 5 Steps - Summery of information entered (With Print & Submit buttons) - Order Complete page (Successfully sent your order message, Send summery in email) The form must be: - Responsive - Must validate the information - Should be Wordpress compatible I need only working form for now. Once it's approved I will need to have it transferred in to Wordpress CMS. I'll provide PSD files. NOTE : My Budget is no more than $70 for this. Please do not BID if you do not have any experience in developing huge forms. Give me working links of forms you have coded. I am not looking for Contact forms here.
