Need to create a customer service ticketing system. Users would go to an online form (at computer or on phone) that allows them to enter in information. Reports need to be generated that provides metrics on the data. I have attached a document that will give you an idea of the fields for the online form and the types of reports that I would like generated. The reports should be easily printable in a neat format. Please submit a sample of the online form and report to demonstrate your understanding of the project.
