Build a backend database and website to display information

Overall project: An ultra simple tablet based customer satisfaction survey, with results saved to backend database where they can be used to generate reports. System is to be able to handle multiple user accounts linked to multiple survey locations. This project request: Develop the backend online database and front end reporting website to display survey results. This will be the 'minimum viable product' to enter into testing with, with the intent of building upon this base in the future. Layout needs to be neat and tidy, graphics can simply be place holders. - Database will need to receive data from a yet to be developed tablet app (can also include the app development in your proposal if applicable). Data fields will be "Location", "Date", "Time" and "Score". Score will be a numerical value of 1 to 5. - Website will need to handle password protected accounts. Each account will be linked to one or more 'survey locations' and only able to see the results from their locations. - Results page is based on a single searchable days date. It will display the average score from that day at that location, as well as a chart of how many scores of 1, 2, 3, 4 and 5 were received, plus a chart for the current weeks daily averages and previous four weeks daily averages. Please see mock up results page attached as PDF. - System also needs to automatically email this daily report as an HTML email to the user. - System needs to have an admin account capable of creating, pausing and deleting all other user accounts, linking user accounts to survey locations, and a report that shows the number of results received for each and every location per month.
