Build a Website

I need somebody who can make some codes for me. I need: 1. a file upload form for images (jpg, png, gif) and videos (mp4) where the user can also add a title, description and add keywords. When it submits, the files will be given a unique number and uploaded and the information (title, description, keywords and file location) will be stored in a mySQLi database. 2. a code that checks if a sessions exists and equals to a value. 3. a download code which can prompt an automatic download once clicked of any file designed within the code. Note, you should consider this a trial run - If you impress me, their will be plenty more work.
