Add Prescription Glasses to Eyewear Frames Ecommerce Site

We have a website [login to view URL] and where we sell Frames (Eyewear). But we need to add prescription lenses to the site. We need someone to do something almost exactly to this how it is on this website - [login to view URL] These are the requirements for the prescription in the site: Basically you need to COPY [login to view URL] prescription option. I will try to list it step by step: Capture – it shows the customer that the price includes lenses and then he can click on “CHOOSE LENSES”. Capture_1 – it asks the customer – what do you use your glasses for? And those options here should be exactly the same. Capture_2 – “ENTER YOUR PRESCRIPTION” – those options here should be exactly the same. Please NOTE this : The Axis opens up only when the customer enter Cylinder If the customer puts the cylinder without the Axis then the system should give an error message PD - start from 40 up to 80 - we need to have the option - I HAVE 2 numbers for my PD * NEAR VISION same options just need to add NEAR PD +0.75-3.00 When it's more than 3 - we can provide add ins please call * PROGRESSIVE same options +0.75-3.00 Capture_3: it says something about product TAG – I don’t know what is that maybe you can figure it out. Capture_4 : when the customer add his prescription it opens up the boxes with the packages offered for him – we should have EXACTLY the same packages we will just name them and price them differently. Capture_5: please see the optional packages. When those packages open up it also need to open ADD SUNGLASSES LENSES OPTIONS (Capture_6) with the ONE YEAR WARRANTY option. Rafael- please note VERY IMPORTANT that each option has a ? mark next to it so it explains the customer what is it for- this is really important to add because it might saves a lot of phone calls. Capture_7: Please note that when the customer enter a CYLINDER it only opens the last two packages.
