Design a 4 part submission form

On the "Get Started" page they will need a form that will be a 4 step process. Each step it will take them to another page. Step 1- Need a submission form with the attached information titled "scope" user will fill out all details and click next and it will take them to step 2 Step 2- Will be a submission form collecting the attached details titled "Quiz" user will fill out all details and click next and it will take them to step 3 Step 3 The will be a small section where user will click on one of the Below name's and then it will select that name. The Black Bull Expedition The Romeo & Juliet The Ernest Hemingway The James Bond The Great American The Walt Disney The David Livingstone The Mother Theresa **Please have text after all above names that says the following" Click here to view more details about our trip themes" WHen user clicks "click here" it should pop up an outside window to the following URL [login to view URL] User will select one of the above names then click next and it will take them to step 4 Step 4- Please just have a comment section where it will ask users the following " Please provide any additional details that you feel will help us provide you with the best experience possible" After user enters comments have a Button that says "Complete" Once user clicks complete it will save all that data and email site owner with all of those details. form needs to be mobile responsive and design to match current site.
