Design New Page for existing Template based Website

We are using existing website template for our product and need some one professional to design and code (HTML+CSS) new page based on our screenshot/requirements.
You can get familiar with template we are using by visiting [url removed, login to view]
Page we need to be completely redesigned is shown in "Page [url removed, login to view]" image attached. This page should be redesigned using better UX/UI.
When submitting your offers please include a sample (maybe high fidelity) to make it easier for us to make a selection.
Ideally we would like to establish long term relationship with some one that is good with UI/UX and hopefully HTML/CSS as this is on going effort.
When we need new page following information will be provided:
1. Sample of existing page that needs to be converted in case it already exists;
2. Description of what needs to be shown and what that page should be.
At the end we need to get new page that will seamlessly integrate with our design.
