Creation of Branded, Editable, Dynamic PDF Form

Hi I need someone to create an editable Adbobe PDF form in my corporate colours with my logo. The form need to be able to be saved by the end user. If possible I would also like the following: 1) It to be dynamic, an example of this is if the client indicates in the form that they need three users it creates the relevant places for say 3 people to enter their user names and passwords. 2) This is a like to have but not necessarily a must have. I would like the form that as people enter their requirements it tallies up the cost for them automatically. Please in you application indicate whether the above two points are achievable.
