We are looking for a UI design who can help us put up few html pages using a bootstrap theme we purchased. The pages to be included are as follows:- 1. Signup a) Form 1 - Name,Email and Password b) Form 2 - DOB , Mobile and SSN c) Confirm Account d) Add Fund Source (Credit Card & ACH) 2. Sign-in a) Mobile Number/Username and Password 3. Dashboard a) Show Balance b) List Transactions c) View Transaction Detail d) Send Fund 4. Settings a) Update - Basic Information (Name,Email,DOB & Mobile) b) Verification Page (SSN) c) Index/Add/Delete Credit Cards d) Index/Add/Delete Bank Accounts 5. Forgot Password a) Put your email to receive to change password b) Type your new password 6. Sign-out You must sign a NDA and Consulting Agreement. Please include portfolio. Our budget is around $60.
