Create Bank Statements

I need the pdf file of an existing Bank statement to be converted into a WORKING TEMPLATE where ALL of the data can be changed, removed or added while keeping the same format. Hence, I need to keep the identical Graphics, Format and Template of this statement; however, I want to be able to change the figures and data (name, amounts etc). The file should be created using a software that I can edit (Preferably, MS Word...) Please provide details on which software you will use to create the TEMPLATE. Preference will definitely be given to designers that have a history of excellent completion and high ratings.
