Birthday Party Art / Graphics

I need some childrens birthday party artwork. I will supply our logo and some other various artwork that I've used in the business to give you something to go on. Overview of environment and business: We host birthday parties for pre-teen children. At the party, guests are encouraged to join our Birthday Club by filling out a form and dropping it in a box. Before the end of the party, we draw a card from that box and award a prize certificate. We need a sign (Birthday Club Sign) that hangs above the prize drawing box (Prize Box). The Prize Box needs artwork on all four sides themed with the Birthday Club Sign. Around the table will be sign-up forms (Birthday Club Sign-Up Form) that party guests will fill-out and drop in the Prize Box. When we draw a winner, they receive a coupon good for tickets (Prize Certificate). These four items should follow a festival, birtday party style theme. 1x 4"H x 6"W (Birthday Club Sign-Up Form) Single-sided "Birthday Club" sign-up form that has a space for name, email address, date-of-birth, address, city, state, zip, and Birthday Club Card #. Card will be presented to birthday party guests to fill out and leave with the host. Should be "Birthday Party Colorful" 1x 8.5"W x 11"H (Prize Certificate) - Double sided. Front should be a "200 Prize Ticket" winner certificate. This certificate will be given to the person whose name is drawn. Backside need only be colorful and along the same theme. Should contiue the Birthday Party theme from the postcard above. 1x 13.5"W x 9.5"H and 1x 6"W x 9.5"H (Prize Box) This artwork will be affixed to the Birthday Club Prize Drawing box. The larger piece will attached to the front and back and the smaller sheet will attach to the side. This box is on the party table into which the guests drop their Postcard (from first item). Should indicate to party guests to drop their postcard into this box. 1x 24"W x 10"H (Birthday Club Sign) This artwork will hang above the party table to encourage party guests to fill out the Postcard and drop on the box below the sign. All of these files should be themed together, but need not be identical. I will provide my existing logo and other artwork I've used in this business for your to either use in this assignment or just to help your imagination. I'll print these files to a large-format (Epson 9600) printer, so high quality and high-resolution is necessary. I use Photoshop 9.0 (CS2) and would like your native files so that I can easily make changes or adjustments in the future. If I can provide further details, please don't hesitate to PM.
